  • A Valid DHCC license.
  • Offer letter from the new facility.
  • Approved service in the new facility.
• Required Documents
  • End of Service Letter from original facility.
  • Offer Letter from the new facility.
  • Medical Malpractice insurance to cover the practice under the new facility.​
  • Passport Size photo for license issuance.

  • Innovation fee of 10.00 AED
